Skilled nursing service is provided by Registered
Nurses (RN’S) and Licensed Vocational Nurse (LVN’S).
Registered nurses are responsible for admission,
recertification, post hospital evaluation, transfer
and discharge assessments.
RN’s design patients plan of treatment in collaboration
with other healthcare professionals. RN’s are responsible
for supervising LVN’s, Home Health Aides (HHA’S)
and other ancillary clinical staff.
Skilled Nursing involves
direct patient that include but are not limited to
the following areas:
- Ongoing patient monitoring
- Patient / Family education
- Wound care
- Catheter insertion / management
- I.V. Therapy
- Injections
- Medication management
- Pain management
- Colostomy / Enterostomy Care
- Diabetic management
- Enteral Feeding (Tube feeding)
- Coordination of care
