Medical social work service is provided by a Licensed
Medical Social Worker (LMSW). Follow up visits can
be made by Social Work Associates under the supervision
of Licensed Medical Social Workers.
Our Licensed social workers and Social Work Associates
help to improve or maintain the social, emotional
and physical health of our patients.
They help the patient and their family with identifying
financial and community based support services.
Our social workers help
in eligibility determination and matching each patient
with the programs they
need. Social workers also make referral to relevant
agencies
that have community support services. These services
include, but are not limited to:
- Medication Assistance Program
- Transportation
- Meals on Wheels
- Insurance coverage / special need assessment
Our social workers also help patients with Advanced
Directives such as:
- Durable Medical Power of Attorney
- Do Not Resuscitate DNR
