COMMUNITY CARE PROGRAMS

Community care programs are non skilled services aimed at helping patients meet their daily needs in the comfort of their own home. To be eligible for community care program a person must:

• Have a doctor’s order of medical necessity
• Have a functional disability
• Need at least one personal care task
• Require assistance with activities of daily living ADL

We provide community care services under the following programs:

• Primary Home Care
• Family Care
