All our clinical staff is involved in an ongoing case
management process.
Case management involves utilization review of each patient’s medical
record and plan of treatment. This process helps to ensure the patient
receives the best quality care within the resources available to them.
Our case management team acts as patient advocates,
charged with empowering the patients and their families
by providing them with information to make informed
healthcare choices.
The case management team also makes the relevant
referral to other resources / groups to help patients
obtain assistance.
Our case managers help patients obtain medical
equipment and supplies when ordered by their Physicians
by referring them to reputable Durable Medical Equipment
(DME) companies.
Case management is not a time-limited service, but
is intended to be ongoing, providing clients whatever
they need whenever they need it.
